About the Role

Supportive Bridge Staffing Agency is proud to recruit for Riverbank, a dynamic residential service located in Hull, just off a main road leading to the city centre and within walking distance of a beautiful park.

Riverbank provides residential support for 24 male and female residents managing complex mental health needs. Operating as a vital "step down" service from hospitals or locked rehab units, as well as a "step up" service to prevent relapse and hospital readmission, our team is fiercely dedicated to helping residents safely reclaim their independence in the community.

Our service supports individuals who may present with:

Complex needs
A forensic background
A dual diagnosis (including a secondary diagnosis of a learning disability or autism)

Working a structured pattern of three 12.5-hour shifts per week across days and nights, you will join a passionate team using personalized, proven therapies to help residents build a healthier, more fulfilling future.

Key Responsibilities

Rehabilitation Support: Assist qualified clinical staff with the assessment and implementation of individual care plans, undertaking routine therapeutic tasks to facilitate recovery, dignity, and treatment.
Person-Centred Care: Provide exceptional personal care and support with daily living tasks, championing resident choice and involvement.
Key Working: Act as a dedicated key worker to help residents set milestones, track their personal progress, and confidently transition toward independence.
Community Integration: Encourage and assist residents in participating in social activities and engaging with community life.
Accurate Record Keeping: Maintain precise care plans, daily charts, notes, and progress logs.
Safety & Compliance: Uphold strict safety, security, and confidentiality regulations in line with care standards.
