As a Care Team Leader, you will oversee daily shift operations, coordinate care plans with multidisciplinary professionals, and lead care assistants on the floor. Key Responsibilities

Lead and supervise shift staff, allocate duties, and ensure the accurate administration and recording of medication.
Case manage residents, conduct risk assessments, and collaborate with families and healthcare professionals to optimize care plans.
Work closely with the senior management team to develop care services and maintain a high standard of person-centered care.

Requirements

NVQ Level 3 in Health and Social Care is preferred, alongside proven experience in leading and supervising care staff.
Strong organizational skills, digital literacy, and a thorough understanding of care planning processes.
Excellent leadership abilities with a genuine commitment to delivering compassionate, resident-focused care.
