About the Role

Supportive Bridge Staffing Agency is partnering with University Hospitals Birmingham (UHB) NHS Foundation Trust to recruit an organized and proactive Band 4 Patient Flow Co-ordinator to join our Urgent & Emergency Care team within Solihull Community Services.

In this vital 12-month fixed-term role, you will work within a dedicated multidisciplinary framework to support Care Coordination and In-Reach Nursing teams. Your efforts will directly help deliver high-quality care closer to home, prevent unnecessary hospital admissions, and ensure seamless, safe patient discharges across the community.

Key Duties & Responsibilities

Patient Tracking & Flow Coordination: Track patients throughout their care journey, ensuring referrals, admissions, discharges, transfers, and ongoing care plans are accurately recorded and managed.
Multidisciplinary Support: Maintain smooth operational flow by attending daily handovers and MDT meetings, supporting waiting time standards, and sharing key information for timely clinical decision-making.
Stakeholder Communication: Act as a professional, compassionate, and approachable point of contact for patients, families, carers, and internal/external partner organizations.
Record Management: Maintain precise patient records using NHS information systems (such as PAS) while upholding strict data protection and confidentiality standards.

Person Specification

Qualifications (Essential)

Knowledge of administrative and operational procedures, specialist IT systems, and information management, acquired through formal training or relevant experience to diploma-level equivalent.

Experience (Essential)

Proven administrative or operational support experience in a patient-facing role within an NHS provider organization.
Experience collaborating within a multidisciplinary healthcare team, acting as a key point of contact for enquiries from patients and stakeholders.
Familiarity with NHS patient information systems, clinical terminology, patient flow/bed management processes, and data protection requirements.
