Senior Social Worker

Supportive Bridge is recruiting on behalf of Elysium Healthcare for a Senior Social Worker to join the team at St Mary's Hospital in Warrington, Greater Manchester.

This is an exciting opportunity for an experienced Social Worker to support individuals with acquired brain injuries, neurological conditions, autism spectrum conditions and mental health needs. Working as part of a multidisciplinary team, you will play a key role in delivering person-centred care, managing complex cases and supporting service users throughout their recovery journey.

Key Responsibilities

Manage a complex caseload and complete social care and risk assessments.
Coordinate admissions, discharge planning and pre-admission assessments.
Liaise with local authorities, healthcare professionals and external agencies.
Support service users and their families throughout the care planning process.
Prepare reports for Mental Health Review Tribunals and attend hearings.
Facilitate access to housing, welfare rights, advocacy and community support services.
Maintain accurate electronic records and documentation.
Provide guidance and support to junior team members.

Requirements

Degree in Social Work, CQSW, CSS or DipSW qualification.
Relevant experience in a similar healthcare or social care setting.
Strong communication and case management skills.
Ability to work effectively within a multidisciplinary team.
Willingness to complete Breakaway and MVA training.
