Supportive Bridge Staffing Agency is proud to partner with Bradford Council to recruit a passionate and experienced Community Care Officer to join the dedicated team within the Short-Term Support Service at Airedale Hospital.

Working within integrated multidisciplinary teams alongside health colleagues and qualified social workers, you will assess needs, safeguard older people, and develop effective, person-centred support plans.

About the Role

As a Community Care Officer, you will promote the dignity, rights, and individuality of the people you support while putting person-centred values into practice. Key responsibilities include:

Safeguarding & Assessment: Understanding and responding proportionately to harm or abuse, assessing risk, and building emotional resilience.
Collaboration & Partnership: Working in line with local policies and collaborating effectively within integrated multidisciplinary teams and external partners.
Legal & Statutory Frameworks: Applying the Care Act 2014, Mental Capacity Act, and relevant safeguarding legislation while respecting joint procedures.
Proactive Support: Communicating clearly to aid joint decision-making, offering solution-focused guidance, and helping individuals plan for their future wellbeing.

About You

We are looking for an enthusiastic, adaptable professional with a positive attitude toward change. Essential requirements include:

Essential Experience: Proven experience in adult services, specifically within assessment and care planning.
Essential Knowledge: Sound knowledge and understanding of the Care Act 2014 and the Mental Capacity Act.
Requirements: Ability to pass an enhanced Disclosure and Barring Service (DBS) check and hold a current driving licence (with appropriate business use insurance, unless a disability prevents this).
